Important Notice Regarding Form 1095-C

As part of our ongoing compliance with IRS regulations, Highpoint Community Bank is providing this notice to inform all employees and covered individuals of their right to receive a copy of Form 1095-C.

What is Form 1095-C?

Form 1095-C is available in UKG for any employee or former employee that has access. Alternatively, if you would like to receive a copy of your Form 1095-C, you may request it at no charge using one of the following methods:

We will process your request within 30 days.
